Aortic Valve Surgery Mini-AVR with Right Anterior Mini Thoracotomy RAMT

Objectives: Aortic valve replacement over a mini thoracotomy was a complex procedure in the past, with the introduction of balloon expandable aortic valves the mini-AVR over a RAMT has become far more reproducible. We retrospectively examined the outcomes of mini-AVR, using the Edwards Intuity valve in 60 Patients. Methods: Between November 2016 and December 2017 a total of 60 Patients underwent isolated aortic valve replacement using the Edwards Intuity valve with a mini thoracotomy. Primary study endpoints were hemodynamic performance during follow-up, including maximum pressure gradient and effective orifice areas. Secondary study endpoints were postoperative outcome within 30 day and survival. Results: Demographics showed a mean age of 76,05 years, 48% were female and the mean risk profile EuroSCORE-II was 1,28%. Mean Bypass time was 81 minutes, aortic clamp time was 50 minutes and mean reperfusion time was 16 minutes. Mean hospitalization was 14 Days. CONCLUSIONS: The present small single-center study is one of the first to evaluate the mini-AVR with RAMT. It shows optimal hemodynamic results with a low incidence of aortic regurgitation.
